*IT IS EXTREMELY IMPORTANT THAT YOU COMPLETE THE SUPPLEMENTARY INFORMATION FORM AND RETURN IT TO EACH SCHOOL YOU HAVE APPLIED TO ALONG WITH A COPY OF YOUR CHILD'S BIRTH CERTIFICATE AND BAPTISMAL CERTIFICATE.
